You should try to answer all of the questions on the form if you can. Some questions are mandatory, and these are marked with an *. Where a question gives answer options, you should choose the option that most nearly matches your application.

Some questions indicate an expected number of words for your answers. No credit will be given for longer answers. You may be prevented from entering more than a certain length of text in some fields.

Please enter all currency in full pounds only, without any pound signs, full stops or trailing zeros (i.e. 249 rather than 249.50, £249 or 249.00)

A1.* Name of Organisation – The name of the organisation making the application. In the case of consortiums please give the name of one organisation that is prepared to take responsibility for the application and any subsequent grant.
   
A2.* Address of Organisation – Please enter the address of your organisation including the full postcode. Please also give the main telephone number for the organisation and a website address if you have one.
   
A3.* Name of Contact Person – Please enter the name of the person responsible for the application. Please also enter the telephone number and an email address you wish us to use in connection with this application. B1.* Briefly describe the aims, objectives and activities of your organisation – Please tell us what your organisation was set up to do. You will find this in your constitution. Please also tell us what your main activities are.
   
B2.* Please give examples of your recent achievements – Why are you the right organisation to carry out this work? In this question we are looking for evidence that you have a good track record and that you are the right organisation to carry out the project / activity that you want us to fund. (Approximately 100 words – maximum 600 characters)
   
B3.* Is your organisation part of a larger organisation? – In our guidelines we state that we will only consider national organisation's appeals at branch level. If your organisation is part of a large national organisation we want to know how you are financially independent of the larger organisation. For example, do you have your own separate accounts and governance? (Approximately 30 words – maximum 255 characters.)
   
B4.* What was your organisation's total annual income as shown in your last set of accounts? £0 - 50K, £50 - 100K, £100 - 250K, £250 - 500K, £500K - 1M, £1M - 5M, £5M or over. Please select the income band from the dropdown that most closely matches.

C1.* Purpose for which funds are requested – This should be a short project title / activity description of what you require funding for. Please describe this in no more than 15 words.

C8.* What organisations provide similar activities in your area and has the charity linked with these or other service providers? – In this question we want to check that the service / activity you propose is needed in your area and that you are aware of other activities that could impact on your work. With some projects it is also important for organisations to demonstrate that they have linked in with other key organisations, for example to take or receive client referrals.
